What is Document Upload & AI Analysis?

AuditCore's Document Upload and AI Analysis engine is where you upload every existing file for a participant — Behaviour Support Plans, shift notes, incident reports, medication charts, previous support plans, handover notes, clinical assessments, occupational therapy reports, and risk assessments. The AI reads all uploaded documents together — not one at a time — and surfaces insights across the full document set. It identifies clinical risks that may be buried in a 40-page clinical report, behaviour patterns spread across months of shift notes, medication interactions documented in separate charts, and compliance gaps against the NDIS Practice Standards. What would take a senior clinician or coordinator hours to extract manually is surfaced in seconds.

Why It Matters

Critical participant information is consistently buried in paper files and PDFs that workers rarely have time to read in full — particularly for complex, high-support participants who may have years of documentation from multiple providers. Clinical risks get missed in handover. Behaviour patterns go unrecognised because no one has time to review months of shift notes. Compliance gaps stay hidden because the connection between a clinical report and a missing element in the support plan is never made. How AuditCore Helps

Select a participant in AuditCore and go to the Document Upload section. Upload any combination of PDFs, Word documents, images, or text files for that participant. AuditCore processes the full document set and generates four outputs: a clinical risk summary covering medication interactions, mealtime risks, seizure triggers, and fall risks; a behaviour pattern analysis identifying triggers, antecedents, consequences, and frequency patterns from shift notes and incident records; a compliance gap report mapping the uploaded documents against Practice Standard requirements and flagging missing elements; and an auto-updated participant profile with key data extracted from the documents. All analysis results are stored and searchable — and feed directly into document generation and support planning.
